About your eye examination appointment
Please arrive 10 minutes before your scheduled appointment time so you can check in with the staff and your information can be updated. Bring all vision and medical insurance information with you, including your current cards.
If you need to cancel your appointment, please try to give us a 24 hour notice as we have a list of patients who are trying to get into our office as soon as possible. We realize that emergencies or circumstances may arise that are out of your control. Thank you in advance for your cooperation.
Contact Lens Patients
If you wear contacts, you need to come in wearing your lenses for a minimum of 2 hours prior to your scheduled appointment time. If you do not have the lenses in, the Doctor will not be able to update your contact lens prescription. In such cases, you would need to return to the office with your contact lenses in for a brief appointment to complete the
exam.
If you are a new patient, please bring in a copy of your contact lens prescription or the contact lens boxes or packets that the lenses come in. If you do not have this information, please contact your previous Doctor for it or provide our office with the location of your last exam.
