Request for Medical Records Policy and Information
Dear Valued Patient:
If you need a copy of your records or them sent to another doctor, we are happy to provide this service for you. Please be advised that your request will take anywhere from 5-10* business days to process.
Due to HIPAA regulations compliance, we are NOT able to provide records on demand upon walk-in or over the phone. Please note that we legally only maintain records for a period of 5 years from the last date of service. If you are outside of that time frame, your records have been securely destroyed in compliance with HIPAA.
Records requests may be subject to a processing fee not to exceed $8. This fee is to cover costs of labor, supplies, preparation and submission or mailing of your records.
*If your record is more than 2 years old or has been transferred from another office location, the processing time may take up to 30 days, as files are moved to secure storage off-site and must be retrieved in order to process your request. We appreciate your understanding and thank you for choosing us for your vision care needs.
**If you are only looking to receive a copy of your glasses or contacts prescription, please note that most prescriptions are valid for only one year. If your prescription is expired, we cannot issue it to you again, as it is no longer valid. If it has been longer than a year since your last exam, your prescription is likely expired and you would need to obtain a new annual exam for an updated prescription. You are welcome to schedule an exam with us by submitting a request here on our website or calling our office for an appointment.
