You may use this form to request an appointment with us. 

We will do our best to accommodate your requested day and time on exam days. 

Our Normal* Business Hours are as follows:

MONDAYS:        11AM-2PM, Optical store open & Administrative services available

                             Exams available by appointment only between 11:00AM-3PM

TUESDAYS:          NO exam appointments, 11AM-2PM, Optical store open & Administrative services available

WEDNESDAYS: NO exam appointments, 11AM-2PM, Optical store open & Administrative services available

THURSDAYS:    11AM-2PM, Optical store open & Administrative services available

                            Exams available by appointment only between 11:00AM-3PM

FRIDAYS:           11AM-2PM, Optical store open & Administrative services available

                             Exams available by appointment only between 11:00AM-3PM

SATURDAYS:    CLOSED

SUNDAYS:         CLOSED

Please note this is an appointment request, NOT an online booking/scheduler.

**Your appointment is not booked until you get a confirmation from us!** 

We will contact you by email to confirm your appointment or reschedule if your requested appointment time is no longer available. A reply email will come from [email protected]. Please take note and keep watch in your email, including spam box, for an email from that address.

If one of the appointment times you've selected is still available, you will receive confirmation via phone or email. We may reach out to schedule another time if the one you've selected is no longer available. If you DO NOT receive a confirmation, please do not assume that an appointment has been set for you. If you do not receive a confirmation within 24 hours of your requested appointment, it has not been set and please contact us back for further scheduling options.

*Please note: Online appointment requests are NOT booked appointments, but only requests. You MUST receive an email or phone call follow up with an appointment confirmation for a booked appointment.
