Not all providers of care are registered under all plans accepted.
Certain insurances may also require special paperwork or a referral specific to your visit. Such authorizations or referrals will be the responsibility of the patient. If you have questions regarding your need for a referral please contact your primary care physician or insurance provider.

Notice of Payment Policies and Procedures:
PAYMENT POLICY: Payment for professional services is required at the time of your appointment. For your convenience, we accept major credit cards, checks, cash or for select services we do accept care credit. www.carecredit.com
MEDICAL BENEFITS ASSIGNMENT AND RELEASE OF MEDICAL BENEFITS INFORMATION AGREEMENT: At the time of my appointment I will request payment of my authorized insurance benefits be made for charges on my behalf to Charles A Garcia, MD, P.A. for any unpaid medical procedures performed now or in the future. I also authorize Charles A Garcia, MD, P.A. to release medical information to my insurance company(ies) or agent now or in the future, for claim consideration purposes. I understand that payment for services does not ultimately remain my responsibility.
NON-COVERED SERVICES: The filing of a claim for any service rendered DOES NOT GUARANTEE PAYMENT from any insurance company. As the patient, it will be your responsibility to pay for these services not covered by your plan. Furthermore, if you have more than one plan (Medicare and AARP even a third plan like Mutual of Omaha) this DOES NOT guarantee your services are covered at 100%. Secondary insurers will generally pay as a what your primary carrier does not cover, but there may still be an out of pocket co insurance, co pay, or deductible. As a courtesy, we can bill the other plans if you provide us the information prior to your visit. You are responsible for any balances not covered after your insurances(s) have paid for their portion.
DIVORCE DECREES: Our offices are NOT a party to your divorce decree. Guardians of minor patients are responsible for the bill at the time of service. The responsibility for minors rests with the accompanying adult and cannot be billed after services have been provided by the practice.
MINOR PATIENTS: For unaccompanied minors, any non-emergency exam or services will be denied unless charges have been pre-authorized to an approved credit card or payment by cash at the time of service has been verified. You must also provide a signed approved form for treatment and examination of the minor prior to the appointment or the exam may need to be rescheduled.
