For your convenience, our patient welcome and history forms can be found by clicking this link .
We encourage you to complete your forms online and submit them to us electronically. Alternately, you can choose to complete the forms when you arrive at the office and we ask you to arrive 10 minutes early to do this and bring with you a list of your current medications.
When you arrive for your appointment please bring all of your insurance card(s), including any vision and medical insurance cards. Please bring all prescription eyewear you may have, including sunglasses, computer glasses, or glasses used for hobbies/sports.
Our office is located at 7629 Melrose Ave LA Ca 90046, across from Starbucks. Parking is available at meters on Melrose, and there also is 2 hour Street parking on Stanley north of the alley.

In an effort to make you more comfortable when you arrive at our office and to streamline the new patient process, you will be able to complete all the necessary items prior to your first visit.
