Making an Appointment

To make an appointment, please either call the office at 617-426-0370 during our regularly scheduled hours or fill out the form on the Contact Us page. You will be contacted the next business day.

For existing patients, we will need the following information to set up an appointment:

• Name of patient
• Name of requested doctor
• Requested date of appointment
• Phone number  during the day or email address
• Insurance provider and assigned insurance ID number
• Insurance referrals, when required

For new patients, we will need the following information to set up an appointment:

• Name of patient
• Requested date of appointment
• Home address
• Date of birth
• Home and business phone numbers
• Email address
• Insurance provider and assigned insurance ID number
• Insurance referrals, when required
